Camp Bilgola Swimming/Water Activities Permission Form

This form MUST completed as it serves as a permission to partake in swimming activities

I hereby give permission for my child to participate in planned swimming activies according to the Camp Bilgola Program

My child's swimming ability is:

POOR* (Struggles to swim more than a few meters independently)

AVERAGE (Comfortably swim for 25m or more, in at least one stroke)

GOOD (Can swim 50m or more, in multiple strokes)

EXCELLENT (Effortlessly swim 100m or more, in multiple strokes)

My child requires a swimming aid:

If answered YES. You must provide this aid to centre staff on the day of the excursion, along with instructions for use. Failure to do so may result in your child not being allowed in the water.

If answered NO. Unfortunately your child will not be able to participate in any swimming acitivies.

*NOTE children must be at least an average swimmer to participate in any swimming activies.
